Apply to become a volunteer mentor

Mentors guide young people (14 - 19 years old) at critical times of their lives. They instill confidence and encourage them to achieve their full potential.

All this by giving one hour every 2 weeks (15 hours of mentor time per year).

You will need to complete:

a) A Leeds Mentoring application form
b) A Criminal Records Bureau (CRB) Disclosure Form

Notes

1. Page 1 of the Mentor Application Form is for administration use only and will not be revealed to the student.
2. Page 2 is information which will be shared with your student.

Do's

1. Use black ink and BLOCK CAPITALS throughout
2. Use only one letter/number for each box. Leave an empty box between words but NOT between postcodes or Tel. No's.
3. Mark choices in boxes with a X not a tick
4. Section H item 68/69 keep your signatures in the box

Sections A - Z

1. Section A Complete boxes 1 - 12 including your Nat. Ins. Number
2. Section B Box 13 Position applied for - Volunteer Mentor Box 14 Organisation Name - Leeds Mentoring, Elmete Centre, Elmete Lane, Leeds. LS8 2LJ
3. Section C and D complete if appropriate
4. Do not complete sections E, F and G
5. Section H Complete and sign both boxes
6. Do not complete sections X (see below for section X) Y and Z

Please bring your CRB form to mentor training or induction.

Section X - Confirming your identity

Leeds Mentoring staff need to confirm your identity.

You will need to provide original identity documentation.

Acceptable identity Documents

GROUP 1

• A Valid passport
• UK driving licence
• UK Birth certificate
• Valid Photo ID card (EU countries only)
• UK fire arms licence.

GROUP 2

• P45/60 statement
• Bank, building society, store card or credit card statement (or similar)
• Utility bill
• Any insurance certificate
• Car registration document
• Exam Certificates
• Valid TV licence

You MUST produce one document from group 1 and two from groups 1 and 2.
